As a Home Health PT, you will:
- Providing treatment as ordered by the attending physician, dentist, or podiatrist.
- Assisting in developing and updating the plan of treatment or Plan of Care (POC).
- Assisting the physician in evaluating level of function.
- Observing, recording, and reporting information on the patient’s condition to the attending physician, case manager, and in the patient’s health record.
- Advising, consulting, and when appropriate, instructing family and other LORIAN personnel in patient’s therapy program.
- Teaching and supervising other healthcare personnel when appropriate.
- Evaluating the home environment and making appropriate recommendations.
- Participating in in-service education programs.
Qualifications
- Graduate of a physical therapy school approved by the Council of Medical Education of the AMA in Association or its equivalent.
- Licensed or registered to practice as a PT in the state of practice.
- Preferably at least two (2) years of experience as a PT.
- Possesses and maintains good physical stamina and mental health. Presents a pre-employment physician’s health clearance, including negative TB skin tests and/or CXR and other tests as required by specific state statute.
- Has the ability to read and follow written instructions and document care given.
- Is self-directing and has the ability to work with little direct supervision.
- Possesses and maintains current CPR certification.
- Is flexible and cooperative in fulfilling the role obligations.
- Demonstrates tact, patience, and good personal hygiene.
- Is a licensed driver and has an automobile that is insured in accordance with Lorian Health (LORIAN) requirements.
- Must be able to verbally communicate in the language of the client and document all client-related activity practices.

- Completes, maintains, and submits accurate and relevant clinical notes regarding patients/clients condition and care given in accordance with LORIAN policies and procedures.
This position is based on a Pay per visit compensation model.
Pay per visit ranges from $100 to $150 visit commensurate with experience and depending on type of visit.
