We are a neurologic specialty outpatient practice focused on helping individuals with Parkinson’s, Alzheimer’s, and similar neurologic conditions maintain mobility, independence, and quality of life.
This is a mobile outpatient therapy position (not home health) where therapists treat patients in their homes and community settings.

About the Role
This role focuses on rehabilitation for patients living with:
Parkinson’s disease
Alzheimer’s disease
Dementia
Movement disorders
Balance and gait impairments
Neurologic mobility limitations
Cognitive and functional decline
You will provide outpatient-style therapy in patients’ homes or community settings, allowing treatment to focus on real-world mobility and function.
Experience with LSVT BIG, LSVT LOUD, or similar neurologic rehabilitation approaches is recommended.
Therapists interested in developing expertise in Parkinson’s therapy, movement disorder treatment, balance training, and cognitive rehabilitation are strongly encouraged to apply.
Responsibilities
Evaluate and treat patients with neurologic and movement disorders
Provide mobile outpatient therapy in patient homes or community settings
Document visits using our EMR system
Collaborate with physicians and care teams as needed
Position Details
1099 Independent Contractor
PRN / flexible scheduling
Competitive per-visit compensation
Administrative team manages scheduling, referrals, billing, and insurance
Qualifications
Licensed Physical Therapist in the state
Interest in neurologic rehabilitation
Experience treating Parkinson’s disease, dementia, or neurologic conditions preferred
Experience with LSVT BIG or LSVT LOUD is a plus
Ability to travel locally to patient homes
