Location: Sacramento, South Sacramento, Elk Grove, CA
Schedule: Full-Time
Compensation: Per-Visit Pay Structure
We’re seeking a compassionate and motivated Physical Therapist to join our growing team of clinicians dedicated to delivering exceptional, patient-centered care in the comfort of patients’ homes. What You’ll Do
- Conduct initial patient visits, including comprehensive assessments and evaluations
- Develop and implement individualized treatment plans
- Supervise and collaborate with Physical Therapist Assistants (PTAs)
- Coordinate care with interdisciplinary team members

What You Bring
- Current, active Physical Therapist license in the state of employment
- Valid driver’s license, auto insurance, and reliable transportation
- Current BCLS/CPR certification
- Home health experience preferred, but not required
- Ability to pass background check, drug/alcohol screening, and OIG screening
