About the Role:
ADDAX HOME HEALTH LLC is looking for a passionate and dedicated Weekend Physical Therapist to join our growing team in Fort Myers, FL! Responsibilities:
- Conduct comprehensive physical therapy evaluations and assessments for home health patients
- Develop and implement individualized treatment plans to restore function, mobility, and independence
- Perform therapeutic exercises, manual therapy, and rehabilitative techniques in the home setting
- Monitor and document patient progress, adjusting care plans as clinically appropriate
- Collaborate with physicians, nurses, and the interdisciplinary care team to ensure coordinated patient care
- Educate patients and caregivers on home exercise programs, safety, and fall prevention strategies
- Maintain accurate and timely clinical documentation in compliance with home health regulations
Requirements:
- Active Physical Therapist (PT) license in the state of Florida
- Graduate of an accredited Physical Therapy program (Bachelor's, Master's, or DPT)
- Home health experience preferred but not required
- Valid driver's license and reliable transportation for home visits in the Fort Myers area
- Strong clinical assessment, critical thinking, and problem-solving skills
- Excellent communication and interpersonal skills with patients, families, and care teams
- Ability to work independently and manage a weekend caseload efficiently
