Key Responsibilities of the Physical Therapist:
- Perform initial and ongoing evaluations of seniors’ physical abilities
- Create individualized treatment plans to improve mobility and function
- Help patients manage pain, regain strength, and maintain independence
- Guide patients through exercises and activities of daily living (ADLs)
- Recommend assistive devices and train patients in their use
- Educate staff and patients on fall prevention and safe mobility practices
- Collaborate with an interdisciplinary care team
- Maintain accurate documentation and progress notes

Qualifications
- Graduate of an accredited school with a bachelors, master’s, or doctoral degree in Physical Therapy.
- License as required by the State of Wisconsin to practice as a Physical Therapist.
