This Full time, Part-time and PRN opportunity is ideal for a physical therapist who values flexible scheduling, clinical excellence , functional evidence based practice and the support of a therapist-owned organization.
As a physical therapist with Greenfield, you’ll work closely with residents to help restore mobility, build strength, and reduce pain. You’ll be part of a collaborative care team committed to improving quality of life through skilled, individualized treatment.

Physical therapist responsibilities:
- Conduct evaluations and develop treatment plans for residents with mobility, strength, balance, and functional limitations
- Provide therapeutic interventions that promote safety, independence, and recovery
- Educate residents and caregivers on proper movement techniques and fall prevention
- Collaborate with a multidisciplinary team to ensure well-rounded, person-centered care
- Monitor and document progress in accordance with regulations and company standards
- Contribute to care planning discussions with insight and clinical expertise
- Help residents achieve the highest possible level of physical function

Qualifications
- Graduate of an accredited school with a bachelor’s, master’s, or doctoral degree in Physical Therapy.
- License as required by the State of Wisconsin to practice as a Physical Therapist.
