Greenfield Rehabilitation Agency is seeking a PRN Physical Therapist (PT) to support our newly acquired senior campuses in Pigeon Falls and Whitehall, WI. This is an excellent opportunity for a dedicated and flexible PT who wants to make a meaningful impact while working within a supportive, therapist-led organization.

Your Responsibilities:
- Perform evaluations and provide skilled physical therapy treatment in a skilled nursing setting
- Develop individualized care plans to improve mobility, strength, balance, and overall function
- Collaborate with the interdisciplinary team to ensure continuity of care
- Complete documentation in accordance with regulatory and company standards
What You Bring:
- Active Wisconsin Physical Therapist license
- Strong clinical reasoning, communication, and organizational skills
- A passion for working with older adults in long-term care environments
- Dependability, flexibility, and a team-oriented mindset

Qualifications
- Graduate of an accredited school with a bachelors, master’s, or doctoral degree in Physical Therapy.
- License as required by the State of Wisconsin to practice as a Physical Therapist.
