Greenfield Rehabilitation Agency is seeking a PRN to Part-Time Physical Therapist (PT) to join our expanding therapy team at our newly acquired senior campuses in Pigeon Falls and Whitehall, WI. This is a great opportunity for a dedicated and flexible PT looking to make a real difference with the potential to grow into a more consistent part-time role.

Your Responsibilities:

Evaluate and treat patients in a skilled nursing setting
Develop individualized care plans focused on improving mobility, strength, and function
Collaborate with the interdisciplinary team to ensure continuity of care
Document patient progress in accordance with regulatory and company standards

What You Bring:

Active Wisconsin Physical Therapist license
Strong clinical, communication, and organizational skills
Passion for working with older adults in long-term care settings
Dependability, flexibility, and a team-player mindset

Qualifications

Graduate of an accredited school with a bachelors, master’s, or doctoral degree in Physical Therapy.
License as required by the State of Wisconsin to practice as a Physical Therapist.
