Position: Full-time or part-time Home Health Physical Therapist (PT)
Location: Thurston County. We are looking for passionate individuals dedicated to providing quality patient care in our new Home Health division.

Responsibilities
Provide comprehensive patient care through assessment, diagnosis, and treatment planning in home health settings.
Develop and implement individualized care plans, focusing on rehabilitation and mobility enhancement.
Educate patients and families on physical therapy techniques and home exercises.
Collaborate with interdisciplinary teams to ensure continuity of care and effective discharge planning.
Maintain accurate documentation of patient progress and treatment outcomes while adhering to medical terminology standards.
Stay current with best practices in physical therapy, particularly in hospice and geriatrics.
Qualifications:
Licensed Physical Therapist in Washington state (or before your first day of work)
One to two years of experience as a Physical Therapist
Current BLS certification
