Overview

Interim HealthCare of Montgomery County is seeking a compassionate and skilled Physical Therapist (PT) to join our growing hospice team. This role focuses on improving quality of life, comfort, and functional mobility for patients receiving end-of-life care in the home setting.

Responsibilities

Perform initial and ongoing physical therapy assessments for hospice patients
Develop and implement individualized care plans focused on comfort, safety, and mobility
Provide therapeutic interventions including positioning, transfer training, and pain-relief techniques
Educate patients, families, and caregivers on safe mobility and home care strategies
Collaborate with the Interdisciplinary Group (IDG) including nurses, social workers, and physicians
Maintain accurate and timely clinical documentation in compliance with regulatory standards
Participate in care planning and team meetings as needed

Qualifications

Graduate of an accredited Physical Therapy program
Active Tennessee PT license (or license eligible)
Hospice or home health experience preferred, but not required
Strong clinical judgment with a patient-centered, compassionate approach
Excellent communication and caregiver education skills
Ability to work independently in a home-based setting

Physical Requirements

Ability to assist patients with mobility and transfers
Prolonged standing, walking, and occasional lifting as required
