Acclaim Home Health is seeking a dedicated and compassionate Physical Therapist to join our Home Health team in Worcester, MA. This role involves providing high-quality physical therapy services to patients in their homes, helping them recover and improve their physical function in a comfortable and supportive environment.

Responsibilities

Evaluate patients' physical conditions and develop individualized treatment plans.
Provide skilled physical therapy services in patients’ homes, focusing on rehabilitation, mobility improvement, and pain management.
Monitor patient progress and adjust treatment plans as necessary to achieve optimal outcomes.
Collaborate with physicians, nurses, and other healthcare professionals to ensure comprehensive patient care.
Document patient evaluations, treatments, and progress notes thoroughly and accurately.
Educate patients and their families on exercises and activities to promote recovery and avoid injury.

Requirements

Licensed Physical Therapist in the state of Massachusetts.
Experience in home health or outpatient physical therapy preferred.
Strong communication and interpersonal skills.
Ability to work independently and manage time effectively.
Valid driver’s license and reliable transportation to visit patients’ homes.
Current CPR certification.
