Description
PRN position available for primarily day shift and some weekends.
We are seeking a Physical Therapist to join our Therapy Services team.
The PT shall screen, evaluate, re-evaluate, discharge plan and make revisions in any established plan of care as supervised by the physician
- Evaluate residents per MD order and establish appropriate Treatment Plans and Goals that meet the specific needs of the individual.
- Re-assess patients and caseload as needed in order to determine need for continued services or discontinuation of services based on patient progress. Carry out treatment plan as indicated.
- Supervise treatment implementation carried out by Licensed Physical Therapy Assistants and determine appropriateness, as well as modify treatment plan as indicated.
- Provide thorough discharge planning that will allow the patient to function at a maximal level of independence in the least restrictive environment.
- Educate staff, family, and care givers as indicated.
- Identify needs for program development and implement programs according to company policies and procedures.
- Identify equipment needs of Physical Therapy Dept. and order equipment according to company and facility policy.
- Complete all required documentation according to guidelines specified in Documentation Manual.
- Coordinate in-services for facility staff as need is determined, to ensure safe and effective carry over of Maintenance/Restorative Programs and overall call of the residents.
Requirements
- Must have a degree in Physical Therapy from an accredited program
- Active state license required
- Possess current working knowledge of Functional Pathways policies and procedures, clinical programs with application skills to therapy disciplines.
- Ability to supervise PTAs and PT Techs
