Job Summary
Provides direct patient care including evaluation and treatment as outlined by the guidelines of NYS Physical Therapy Practice Act
and Hospital policy.
Provides daily clinical supervision to Sr. P.T's, Staff P.T.'s, P.T.A.'s, Rehabilitation Aides/ Senior Nursing Attendants and students.
Embraces the team concept of the department and the customer service approach to patient care.
Understands work schedule may include rotations, weekend, and holiday coverage.
Essential Functions
- Provides direct patient care including evaluation, treatment, equipment assessment and documentation.
* All patients are evaluated within 24 working hours of receipt of prescription, each patient contact or attempt is documented in the medical record on the same day and equipment is provided or ordered as appropriate 100% of the time. - Maintains own patient schedules and utilizes time efficiently
* Therapist maintains a full patient case load as per service requirements and departmental policy. - Maintains accurate patient records, statistical reports and other documentation related to patient care
* Patient records and statistical data are completed daily 100% of the time.
* Submits statistical composite on a monthly basis. - Performs patient and family education and attends family meetings and educational sessions.
* All patient and families, when available, receive education 100% of the time as evident in daily documentation. - Participates in departmental performance improvement activities, program planning and equipment needs.
* Therapist provides performance improvement data to supervisor on a monthly basis, discusses program development and recommends equipment for departmental use. - Attends departmental educational activities including inservices and continuing education courses and interdisciplinary conferences. Provides formal inservice presentation to physical therapist staff and various hospital personnel.
* Attends at least one or two outside courses/conferences annually and provides inservices in all completed conferences. Attends 100% of schedule inservies and Interdisciplinary Conference meetings. - Provides direction, instruction, and review of work performed by Sr. PT's, staff PT's, PTA's and Senior Nursing Attendants/Rehabilitation Aides.
* Therapist monitors patient care related duties of individual staff member on a daily basis and discusses treatment and work performance. - Acts as the Clinical Coordinator of Clinical Education (CCCE) and provides mentoring to clinical instructors and supervision of affiliating students.
Maintains an active student program with at least 8 students scheduled annually.
* Acts as a mentor to clinical instructors.
* Supervises at least one student. - Demonstrates awareness of Infection Control Policies and safety procedures in both patient care and physical environment.
* Therapist immediately notifies the supervisor of equipment malfunctioning and environmental problems. Follows Infection Control policy 100% of the time. - Assumes responsibilities for requirement of various services regarding scheduling, assignment and coordination of patient care and statistical reports.
* Therapist completes daily overall patient care activities and submits statistical composite to Associate Director on a monthly basis. - Acts as a clinical resource for physical therapy staff in more complex treatment techniques and provides expertise in developing protocols and treatment care plans.
* Therapist guides and trains in more advanced patient care skills on a daily basis or as needed.
* Therapist provides input to Associate Director in program development as warranted by department's needs. - Assists in the organization and coordination of in-service programs and acts as chairperson in the physical therapy education committee.
* Therapist maintains ongoing in-service schedule. - Maintains professional behavior in all patient and staff interactions.
* Therapist exhibits the ability to interact with all staff and patients. - Core Team members will actively participate in all aspects of the interdisciplinary care model.
* Therapists will attend daily meetings to communicate with team members regarding rehabilitation assessments, goals, progress, and outcomes. - Non-core team members will actively participate in interdisciplinary care model activities as necessary.
* Therapist will attend daily meetings as necessary as stated above.
Qualifications
• Graduate of a program in Physical Therapy approved by the American Physical Therapy Association
• Current New York State Licensure
• Two (2) years at senior level in field or acute medical hospital
• Two (2) years supervised clinical experience in a medical setting preferred, or equivalent combination of education and experience
• Must have a valid driver’s license and ability to provide own transporting during working hours for the purpose of making home visits
Hours: M-F 8:30a – 5:00p- some weekends and some holidays.
