VNA & Hospice of the Southwest Region is seeking compassionate and motivated Physical Therapists (PTs) to join our team in Rutland County. We are hiring for full-time and per diem positions working with adult and/or pediatric patients in the home health setting.

POSITION SUMMARY
As a Physical Therapist, you will evaluate patients, develop individualized treatment plans, and provide skilled therapy services in the home, facility, or educational environment. Caseloads may include adult and/or pediatric patients depending on experience and interest.
Responsibilities include:
- Completing PT and OASIS assessments as appropriate
- Developing and implementing individualized plans of care
- Providing therapeutic interventions and caregiver education
- Monitoring patient progress and adjusting treatment plans as needed
- Collaborating with interdisciplinary team members
- Supervising PTAs, students, and home care aides when applicable
- Maintaining timely and accurate clinical documentation
QUALIFICATIONS
- Degree in Physical Therapy from an accredited program
- Current Vermont PT license
- Valid driver's license, ability to successfully pass pre-employment driver record, criminal, adult and child abuse background check/review required.
- Home health or prior clinical experience preferred
