Home Health Physical Therapist (PT)

Billet Health is seeking a compassionate and motivated Physical Therapist (PT) to join our growing Home Health team serving the West Valley area.
Our clinicians provide personalized in-home care while building trusted relationships with patients throughout Arizona, Nevada, California, New Mexico, and Texas.
As a Home Health Physical Therapist, you will provide physical therapy services to patients in their homes in accordance with physician orders and individualized plans of care under the supervision of the Director of Clinical Services/Clinical Manager.
Responsibilities
- Evaluate and assess patient functional abilities
- Develop and implement individualized therapy plans
- Provide physical therapy treatment in patient homes
- Monitor patient progress and communicate updates to the care team
- Educate patients and caregivers on exercises and mobility techniques
- Maintain timely and accurate documentation
Qualifications
- Graduate of an accredited Physical Therapy program approved by:
- American Physical Therapy Association (APTA)
- Committee on Allied Health Education and Accreditation
- Or equivalent accredited program
- Active Physical Therapist license in the state of practice
- Valid driver’s license
- Valid auto insurance
- CPR or BLS Certification
- Current TB documentation
- Skin test within the last year OR
- Chest X-ray within the last five years
Compensation & Schedule
- Pay starting at $80 per visit
- Per Diem opportunity
