What you will do:
- Work closely with patients to assess their physical and psychosocial needs.
- Planning, development, and implementation of multidisciplinary plan of care
- Evaluation of patients' responses to care.
- Maintain patient records including initial evaluations/assessments, progress notes, and discharge summaries.

What you will need:
- Graduate from an accredited program for Physical Therapy
- Current licensure as a Physical Therapist in the State of WI
- CPR Certification (or ability to obtain within 3 months)
