We are housed in a brand-new outpatient gym where our rehab team is committed to community outreach including hosting a monthly stroke support group. We offer one-on-one patient interactions, a strong team culture, rotating weekends, and opportunities for growth and development. We balance new techniques and specialties including dry needling, certified hand therapy, and FEES, with a personalized, community-focused approach to healthcare.
How you will contribute
A Physical Therapist (PT) who excels in this role:
- Performs initial and ongoing assessments of patient condition and functional status.
- Develops and updates an individualized plan of care in collaboration with the patient, based on goals and capabilities.
- Plans, prepares, and carries out evidence-based interventions to maintain, improve, or restore function; alleviate pain; and prevent dysfunction.
- Delivers therapy using standard PT techniques and modalities appropriate to patient age and condition (e.g., moist heat/cold, ultrasound, UV/IR lamps, therapeutic exercise, manual therapy).
- Administers physical agents and operates PT equipment safely; performs necessary set-up and checks.
- Evaluates, fits, and adjusts prosthetic and orthotic devices; recommends modifications as needed.
- Identifies/document goals, anticipated progress, and timelines; plans for reevaluation and discharge.
- Directs and supervises PTAs, students, and support personnel; delegates tasks appropriately and ensures clear communication.
- Accurately documents assessments, interventions, patient response, education, and progress toward goals.
- Participates in performance improvement and continuing education; performs other duties as assigned.

What we are looking for
- A completed a Bachelor’s degree (PT)
- Current VA PT license.
- BLS required within 30 days of hire.
