What You'll Do
As a Home Health Physical Therapist, you will:
- Perform comprehensive initial evaluations to assess patient needs and develop individualized treatment plans.
- Design and implement therapeutic programs that improve mobility, restore function, relieve pain, and prevent further disability.
- Monitor patient progress and modify treatment plans to maximize outcomes.
- Educate patients and caregivers on exercises, safety, and strategies to promote independence.
- Collaborate with physicians, nurses, and the interdisciplinary care team to ensure exceptional patient care.
- Maintain accurate, timely, and compliant clinical documentation.
- Follow infection control and safety guidelines during all patient interactions.
- Deliver compassionate, high-quality care that enhances patients' quality of life.

Qualifications
- Graduate of an accredited Physical Therapy program
- Current, unrestricted Physical Therapist (PT) license in the state of practice
- CPR Certification
- Valid driver's license with reliable transportation and current auto insurance
- One (1) year of Home Health experience preferred
- HomeCare HomeBase experience preferred
Physical Requirements
- Ability to stand, walk, sit, bend, squat, kneel, and climb for extended periods.
- Ability to lift, carry, push, and pull up to 100 pounds with appropriate assistance when necessary.
- Ability to travel throughout the assigned service area to provide in-home patient care.
