POSITION TITLE: PHYSICAL THERAPIST
RESPONSIBLE TO: THERAPY TEAM LEAD & DIRECTOR OF HBSI
EXPOSURE LEVEL: CATEGORY I
JOB STATUS: FULL TIME/EXEMPT
SUMMARY:
Administers physical therapy to patients on an intermittent basis in their place of residence. This
is performed in accordance with physician orders and plan of care under the direction and
supervision of the Therapy Team Lead and Director of Home Based Services.
ESSENTIAL JOB FUNCTION: Responsibilities to Patients
➢ Provides physician prescribed physical therapy.
➢ Improves or minimizes residual physical disabilities of the patient.
➢ Returns the individual to an optimum and productive level within the patient’s
capabilities.
➢ Promotes prevention, health, wellness, and fitness.
➢ Directs and supervises Physical Therapist Assistants according to state guidelines.
➢ Responsible for initial assessment, plan of care development, and functional
reassessments every 30 days.
➢ Consults with physicians regarding patient’s progress and changes in treatment.
➢ Instructs patients and caregivers in home therapy programs.
➢ Participates in Quality Assurance and Performance Improvement activities as
assigned.
➢ Interacts and practices in collaboration with the patient, family, HBSI team, other
healthcare professionals and staff.
ESSENTIAL JOB FUNCTION: Record Keeping
➢ Completes and maintains necessary records, including OASIS assessments,
reassessments, transfers, resumptions of care, discharges, and routine clinical
notes.
➢ Helps develop the plan of care and revises as necessary.

➢ Follows safety policies of St. John's United and HBSI
➢ Cleans and maintains equipment in good working order
➢ Follows infection control policies
➢ Identifies, reports, and/or corrects obvious safety hazards

ESSENTIAL JOB FUNCTION: Employee Responsibilities
➢ Follows policy and procedures on annual TB testing.
➢ Follows policy and procedures on annual influenza vaccinations.
➢ Attends all mandatory in-services.
➢ Performs other duties as assigned by Therapy Team Lead or the Director of HBSI.
REQUIREMENTS:
Minimum Requirements: Must be a licensed Physical Therapist in the State of Montana
preferably with one to two years related experience. Must have a valid driver’s license and
vehicle insurance.

PHYSICAL DEMANDS:
➢ Ability to work for extended period of time while standing and being involved in
physical activity.
➢ Heavy lifting.
➢ Ability to do extensive bending, lifting, and standing on a regular basis.
WORKING CONDITIONS:
➢ Works in patient homes, independent living communities, and assisted living
communities.
➢ Work involves some exposure to hazards, blood borne pathogens, or physical
risks, which require following basic safety precautions.
➢ Work involves exposure to plants and animals within the patients’ home
environments.
➢ Travel to/from patient home environments.
