Join a growing Home Health team that is dedicated to helping patients regain strength, mobility, and independence in the comfort of their own homes. We're seeking a compassionate, motivated Physical Therapist (PT) who enjoys working one-on-one with patients and making a lasting impact every day.
Position Responsibilities:
- Perform comprehensive physical therapy evaluations and develop individualized treatment plans
- Provide skilled physical therapy services in patients' homes
- Help patients improve strength, balance, mobility, endurance, and overall function
- Educate patients and caregivers on home exercise programs, fall prevention, and safe mobility techniques
- Monitor patient progress and modify treatment plans as needed
- Complete accurate and timely documentation
- Collaborate with physicians, nurses, and the interdisciplinary Home Health team to provide exceptional patient care
Qualifications:
- Current Indiana Physical Therapist (PT) license
- Home Health experience preferred, but not required
- Strong clinical assessment and communication skills
- Compassionate, dependable, and patient-focused
- Valid driver's license and reliable transportation
