OverviewDeliver skilled physical therapy services in accordance with the Florida Practice Act and professional standards
• Perform comprehensive patient evaluations and develop individualized treatment plans
• Establish measurable goals and track functional progress
• Educate patients and caregivers to promote safety and independence
• Supervise and support Physical Therapy Assistants (if assigned)
• Maintain accurate and timely clinical documentation
• Collaborate with interdisciplinary healthcare teams
You will practice with clinical autonomy while being supported by the Director of Rehab and a collaborative homecare team.
Must successfully pass an AHCA Level 2 screening for this position.

QualificationsGraduate of an accredited Physical Therapy program (Bachelor’s, Master’s, or Doctorate)
• Current Florida Physical Therapist license (in good standing)
• Home health experience preferred
• Must successfully pass AHCA Level 2 background screening
• Frequent driver (daily travel required; mileage reimbursement provided)
• Valid Florida Driver’s License
This position requires regular travel to patient homes, defined as:
• Driving daily for business purposes
• Five or more trips per week
• Or averaging over 150 miles per week
