As a Home Health PT, you will:
- Evaluate patients in their homes
- Develop individualized physical therapy treatment plans
- Provide therapeutic exercises and functional mobility training
- Address strength, balance, gait, mobility, and safety needs
- Educate patients and caregivers
- Help patients maximize independence and quality of life
- Collaborate with physicians, nurses, occupational therapists, speech therapists, and other members of the interdisciplinary team
- Supervise and collaborate with Physical Therapist Assistants (PTAs)

Physical Therapist Qualifications
- Active, unencumbered Florida Physical Therapist (PT) license
- Physical Therapy (DPT or masters-level PT) degree from an accredited program
- Valid driver's license
- Reliable transportation
- Current auto insurance
- Ability to provide physical therapy services independently in patients' homes
Home Health Experience Preferred – Not Required
Physical Therapists with experience in home health, home care, outpatient, inpatient rehabilitation, skilled nursing, hospital, orthopedic, neurological, geriatric, or acute care settings are encouraged to apply.
