Location: Eastern Suffolk County, including Medford & Patchogue
Job Type: Full-time
Base Salary: $95,000–$110,000/year
Sign-On Bonus: $3,000
Metro is looking for a motivated and passionate full-time Physical Therapist to provide in-home care throughout Eastern Suffolk County.

Responsibilities
Accept and treat patients within your designated geographic area
Evaluate patients’ needs for physical therapy in the home setting and develop individualized treatment plans
Develop a plan of care and treatment goals in consultation with the physician, patient, and family
Plan, prepare, and implement customized therapy programs to restore function and promote independence
Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques
Provide timely and accurate documentation for each visit
Communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning
Deliver care in compliance with agency policies and professional practice standards
Qualifications
Graduate of an accredited Physical Therapy program
Licensed Physical Therapist in the state of New York or eligible for licensure
Home health experience is preferred but not required — we will provide you with a mentor to help with documentation and compliance
Ability to travel to patients’ homes within the assigned geographic territory
Additional Information
The anticipated base salary range for this position is $95,000–$110,000 annually. Salary is based on various factors, including relevant experience, knowledge, skills, other job-related qualifications, and geography.
This position is eligible for a $3,000 sign-on bonus. Additional terms and conditions may apply.
