We are seeking a passionate and dedicated Physical Therapist to join our dynamic healthcare team. In this vital role, it will be mainly evaluations with the potential to grow this into a full time position. You will work closely with our COTA.

Duties
Conduct thorough assessments of patients’ physical, cognitive, and emotional needs using physiology knowledge and medical terminology
Develop and implement tailored care plans that address individual goals for activities of daily living (ADLs), mobility, sensory processing, and functional skills
Collaborate with PT, PTA, and COTA on staff.
Manage discharge planning by evaluating patient progress
Provide case management services for patients in our outpatient clinic
Educate patients and caregivers on adaptive techniques and equipment to support ongoing therapy goals
Maintain detailed documentation of patient progress, treatment sessions, and care plans in compliance with healthcare standards
Requirements
Valid state licensure as an Physical Therapist with current registration or certification as required by local regulations
Proven experience working in outpatient clinics
Knowledge of anatomy and physiology relevant to occupational therapy practices; familiarity with medical terminology essential
Skilled in developing care plans that incorporate patient-centered goals and discharge strategies
Strong case management skills with the ability to coordinate services across multiple providers and settings
Excellent communication skills to effectively collaborate with healthcare teams and educate patients/caregivers
Pelvic health experience is not required but strongly preferred
