Daily Work Times: Mon-Thur: 8:00 a.m. - 6:00 p.m.; Fr: 8:00 a.m. - 12:00 p.m.
Scheduled Biweekly Hours: 80
Position Summary: Performs physical therapy treatments for patients referred to department.Conducts patient evaluation, performs documentation of patient progress, treatment planning and completes discharge functions
Essential Functions and Responsibilities:
1.Employee will demonstrate the process of obtaining history, performing relevant systems reviews, and selecting and administering specific tests and measures to obtain data.
2.Employee will demonstrate the dynamic process in which the therapist makes clinical judgments based on data gathered during the exam.
3.Employee will perform effective treatment addressing the specific impairments identified in the evaluation.
4.Employee will appropriate techniques that are specific to impairments.
5.The employee will demonstrate ongoing re-assessment and identification of progress of the patient.
6.Employee will initiate and manage the discharge plan beginning at the first visit and continue throughout the patient’s treatment.
7.Employee demonstrates legible, accurate and clear documentation within the established time frame according to departmental policies and procedures.
8.Employee demonstrates written and/or dictated documentation in a format that is concise and complete according to departmental policies and procedures.
Required:
Possess 5-7 years formal education and Bachelor/Master Degree/Doctorate in accredited Physical Therapy program.
Licensure in State of Michigan required.
Trained and educated in age-specific categories being served:infant, pediatric, adolescent, adult and geriatric.
Capable of spending 10-25%25 of time lifting, carrying, moving items weighing 25 pounds or more.
Ability to effectively communicate and present self to people in courteous, tactful and poised manner.
Preferred:
Current membership through the American Physical Therapy Association
Required:

Licensure in State of Michigan required.
