Responsibilities
- Perform evaluations and establish treatment goals
- Develop individualized treatment plans
- Provide skilled physical therapy services
- Complete clinical documentation
- Educate caregivers on home exercises

Requirements
Arizona PT or DPT license, CPR certification, AHCCCS ID, Article 9 certification, reliable transportation, and strong clinical reasoning skills.
