Duly Health and Care is seeking a PRN Physical Therapist to join their Rehabilitation Services team in Glen Ellyn, Illinois.

The ideal candidate will have a current Illinois Physical Therapist license and be able to evaluate and treat patients with orthopedic and musculoskeletal conditions.

Responsibilities:

Evaluate and treat patients with orthopedic and musculoskeletal conditions
Develop individualized treatment plans and goals
Collaborate with physicians and rehabilitation colleagues to ensure coordinated care
Document patient care accurately within the EMR
Deliver exceptional patient experiences focused on outcomes and quality care

The role offers a flexible scheduling with a minimum commitment of two Saturdays per month and opportunities for weekday mornings, evenings, and Saturdays.

Benefits:

Anticipated hourly rate of $40.00-$55.00 per hour
$5.00 per hour weekend shift differential for Saturday coverage

Requirements:

Graduate of an accredited Physical Therapy program
Current IL Physical Therapist license, or eligibility to obtain licensure
CPR certification preferred
